If you haven't steam is probably using the Default mic. Also tinker with the settings a bit. Might be in left 4 dead also, there should be a setting that makes the mic enabled when you speak or when you press a button. Change it to "press button to speak". #5.
A good sensitivity is usually around 30-60cm per 360. It depends on your grip (palm, fingertip, etc) and your aiming style (arm or wrist). There are plenty of professional/very high tier players across all types of games who use a very high/very low sens. What you mentioned is in the mid/low range.
In advanced video settings: Display mode = Windowed no border - gives better alt tab and doesn't suffer much fps loss. no weird no texture glitches after alt-tab. Film Grain = off - screen clarity. Anti-Aliasing Mode = off or 2x MSAA at most - unneeded detail. Filter Mode = Bilinear/Trilinear - no reason to run higher than this.
You can then start a lobby that will launch into that server with the console command mm_dedicated_force_servers xxx.xxx.xxx.xxx:xxxxx with the x's being the ip:port of the server you found. Still haven't had an expert come through with concrete facts though : (. set it to 2.0 since l4d2 uses miles audio codec for deciding game sounds. if you want to try and use creatives hardware engine for processing sound via OpenAL then install Creative Alchemy add l4d2 to it (or any source game), set sound to 2.0 in game and add snd_legacy
